Unity Foundation is a community-driven, non-profit organization based in Toronto, Ontario. Unity Foundation was established in response to the growing demand for personalized, culturally competent, and accessible home care services for seniors, persons with disabilities, and other vulnerable individuals across the Greater Toronto Area.
Our core mission is to improve lives by providing dependable, respectful, and high-quality in-home care—enabling our clients to maintain independence, dignity, and wellness in the comfort of their own homes.
We work in close partnership with families, caregivers, government agencies, and health service providers to deliver services that reflect the unique needs and values of each individual.

Our Team
Our team includes:
- Certified Personal Support Workers (PSWs)
- First Aid and CPR trained professionals
- Police background-checked staff
- Workers trained in infection control, elder abuse prevention, and mental health awareness
- Supervised, evaluated, and continuously trained personnel
We match PSWs with clients based on culture, language, needs, and preferences to ensure continuity and trust in care delivery.
Our Care Approach
📋 Intake & Assessment
A comprehensive, no-cost in-home or virtual assessment is conducted to evaluate the client’s medical, emotional, social, and cultural needs.
🗂 Care Plan Development
We create a personalized care plan including goals, service schedules, risk factors, and special needs. Plans are regularly reviewed and updated.
🧑⚕️ Service Delivery
Care is provided as outlined in the care plan. Daily documentation and performance monitoring ensure safety and satisfaction.
💬 Ongoing Communication
Families receive regular updates and can request changes at any time. We prioritize clear, culturally respectful communication.
✅ Quality Assurance
We implement:
• Client satisfaction surveys
• Incident reporting and resolution
• Quarterly service audits
• Ongoing staff training and evaluation
Policies & Safeguards
- Infection Prevention & Control (IPAC): Following Public Health Ontario protocols
- Incident Reporting & Risk Management: All incidents documented and investigated
- Privacy & Confidentiality: Full compliance with PHIPA; all data securely stored
- Complaint Resolution: Accessible process with escalation to Ontario Health if needed

Unity Foundation is registered on the Ontario Tenders Portal and actively engaging with:

We are ready to collaborate as a subcontractor or direct provider of PSW services, with all necessary documentation, insurance, policies, and credentialing in place.
